How to refer
Healthcare Professionals may refer any patient to the Hospice with a life limiting illness (e.g. Cancer, COPD, Heart Failure, Motor Neurone Disease, Dementia).
Who can be referred?
Healthcare Professionals may refer any patient to the Hospice with a life limiting illness (e.g. Cancer, COPD, Heart Failure, Motor Neurone Disease, Dementia).
Referrals are accepted for patients residing in the Tameside and Glossop area.
Who can Refer?
Any Healthcare Professional such as: GPs, Macmillan Nurses, Hospital Teams and Professionals, District Nurses, Long Term Condition Teams.
What can patients be referred for?

How are referrals accepted?
Each referral is assessed individually and discussed at our regular morning meetings (Monday – Friday), where the hospice team will decide which referrals are more appropriate to which service.
